Austin struggles over whether to transplant the heart of a suicide victim into a teenage girl. Dr. Hancock is trapped in a vicious nerve gas attack in a bank. Wilkes and Kronk respond to the scene. Dr. Shutt tries to find a happy medium between psychiatry and neurosurgery. With McNeil's help, Aaron is scheduled to perform his first surgery since the aneurysm. With Diane returning to work, Billy and Diane leave their child with a babysitter, prompting Billy to consider staying home during the day.

Dr. Corcoran (Dr. Death) is diagnosed with pancreatic cancer. Corcoran invites all her friends, and Dr. Grad, to a party where she plans to take her own life. Aaron treats a young Hasidic man with OCD, trying to find a way to tell his father he's gay. Meanwhile, Cacaci extols the virtues of his 'Y2K' or year 2000 plan.

McNeil is roped into coaching a women's basketball team. This episode contains no lectures on the evils of firearms.

Diane, already guilty about leaving little Emily home with Billy, overreacts when a malnourished child dies in the ER, accusing the parents of starving the infant. Aaron gets an opportunity to exercise his psychiatric skills when a patient is brought in complaining of frequent waking blackouts. New doctor Robert Yeats aids Wilkes in the ER, earning first Wilkes' derision with his very unorthodox theories, and then in textbook fashion, his praise.

Meanwhile, new father Billy and Yeats clash over Yeats treatment of an infant.

Aaron treats a manic-depressive 'Brill Building'-era songwriter who's trying to stay off lithium to write one last hit. Dr. McNeil violates ethics when he gives a Jehovah's Witness a blood transfusion after the patient specifically refused one. Cacaci's young son visits the ER, offering opinions and enlightening staffers on exactly what Cacaci thinks of them.

Wilkes brings his son into the hospital after Raymond starts suffering from mysterious asthma attacks, which Diane believes is related to the schoolyard shooting incident.

Austin worries that NASA will reject her when she learns she has an ovarian cyst. Yeats invites her to a yoga retreat in the hopes of clearing her mind. Sara, staying with Jack, stays outside all night worrying about her mother's safety in the spacecraft. Jack finds Sara unconscious outside, suffering from hypothermia.

A sleep-deprived Shutt acts increasingly psychotic, even convincing Phillip that Aaron bears him ill will. Meanwhile, a couple who've just lost their only child try to have him cloned. Jack treats an Alzheimer's patient who mistakes him for a long-lost boyfriend.
